2-Part Oral Drug Test Result Form 25/Package 

The numbered, paper pack is a 2-Part carbonless form with which provides a carbonless copy for the specimen donor.  Twenty-five forms per pack.

How to complete a 2-Part Result Form or Photocopy Template

1. Write in company and donor information.
2. After the specimen is collected and the drug test device is sealed with a security seal the donor signs the form and provides their contact information. At this point a copy of the form is to be given to the donor who now may be dismissed.
3. When the drug test results are available the collector fills in test details such as Lot # and Expiration Date and records the test results.
4. The collector signs the form and records time and date of collection.
